About

I discovered the cause of migraine attacks in 1974 while an under-graduate student in Ajmer, Rajasthan, India. The scientific project was reported to the Rolex Awards Committee in Bern, Switzerland in 1989, while I was working in Dubai, UAE. I have: rationalized the neurophysiology, neuroanatony, and neuropharmacology of migraine; published original major reviews in migraine pathogenesis and pharmacology, original key hypotheses, original editorials, important critiques, and original case reports; invented the manual ocular compression manoeuvre to instantaneously abort migraine headache, cough-induced headache, post-coital headache (primary headache associated with sex) and cluster headache (and all other so-called TACs/primary headaches); evolved the basis and management of cough-induced headache, for which at one one time lumbar puncture and pneumoencephalography was practiced; rationalized use and abuse of magnesium and opioids in clinical practice in cardiology and neurology, respectively; established the role of metoclopramide as a powerful non-opiate migraine aborting agent as well as useful non-opiate cough sedative/suppressant through vasopressin release with antinociceptive, vasomotor and behaviour control. In stark contrast to elevation of systemic blood pressure, vasopressin lowers the intraocualar pressure (IOP) and remits early acute migraine attacks and prevents migraine attacks through amitriptyline. My other key contribution to science also involves the adaptive vasopressin-serotonin-noradrenaline nexus that delays nitroglycerine, adrenomedullin, and amyline analogue-induced migraine-like headaches, limits duration of primary including migraine and cluster headache, and prevents migraine in four-fifths of the general population. My most salient contributions involve the final antidromic (afferent/efferent) pathogenetic link between migraine and the eye with autonomic nervous system-linked ocular choroidal circulation-linked intraocular pressure aberration related nociceptive corneoscleral neural impulses in the ophthalmic division of the trigeminal nerve, complete deracination of both brain-centric theories such as putative cortical spreading depression and the trigeminovascular "system", and a complete overarching robust defensible biologically-plausible and progressively strengthening multi-dimensional cross-disciplinary synthesis of the primary headache spectrum with the brain being replaced by the eye, and, with elimination of the neuronal/vascular/neurovascular hypotheses that have crippled migraine science for the last two hundred years. I predicted the complete failure of use of botulinum toxin for migraine (2006), use of patent foramen ovale closure to prevent migraine (2010), and complete uselessness of CGRP-antagonists to prevent migraine in the real-world (2017). I have rationalized use and misuse of the randomized controlled clinical trial, informed consent, placebo, and statistical mathematics in migraine research, and, published a futuristic book: "Adaptive Mechanisms in Migraine. A Comprehensive Synthesis in Evolution. Breaking the Migraine Code", Nova Science, New York. I have also introduced the use of sub-lingual and lozenge-form of nicotine in the management of migraine and refractory constipation as well as topical ocular hypotensive timolol maleate as well as topical bimatoprost ophthalmic solution therapy in an organized pharmacologically- and biologicallly-plausible management strategy for preventing migraine as well as aborting early migraine headache, with ultimately futuristic ocular surgery to promote ocular outflow of aqueous humour in genuinely refractory migraine patients. I have rationalized the pathophysiological mechanisms underlying post-lumbar puncture headache as well as timely use of the blood patch. I have placed iron-deposition in the brain in biologically-plausible template, moving it away from the pathophysiology of migraine. I have also discovered the link between constipation, nicotine therapy, Vaslalva manuver, and migraine, and the mechanistic manoeuvre to overcome such Valsalva-induced migraine headaches. I have evolved the cranio-ocular mechanisms of managing "stress" and "exhaustion of stress" to provoke migraine attacks and to prevent migraine. I have created the 10 laws of migraine with complete exposition of the disorder. I have challenged use of WHO-recommended magnesium sulphate in eclampsia and underscored the peripheral pseudo-anticonvulsant action of massive doses of exogenous Mg++ and its limited efficacy. I have discovered the use of transdermal / oral (lozenge-chewing gum formulations) in pain refractory to non-opioid analgesics. Cardiac sound selective transmission to carotid-sinus / mandibular region was published. The blue sclera of migraine is fundamental to its pathogenesis. I have created the laws of migraine/primary headache pathophysiology that take the neuroscience of migraine well-into the THIRD MILLENNIUM.
